Your funding level is determined through an aged care assessment.
The Support at Home program includes eight funding classifications, with each year’s budget distributed across four quarterly payments.

In addition to your regular care plan, Support at Home offers adaptable pathways to meet evolving needs.
1. Assistive Technology and Home Modifications (AT-HM) Scheme
Practical home modifications or equipment designed to enhance your safety and independence. Examples include grab rails, ramps, or mobility aids recommended by an allied health professional.
2. Short Term Restorative Care
Receive short-term support around 12 weeks, aimed at helping you bounce back and restore your daily independence following illness, injury, or hospitalization.
3. End of Life, Palliative Care
Specialist care to ensure comfort, support, and connection at home during life’s final stage, with a focus on dignity, choice, and cherished moments with loved ones.

You’ll make a difference by helping older Australians live independently at home - with dignity and joy.
Position Responsibilities:
• Provide companionship, assistance, and support to clients
• Assist with daily activities such as outings, shopping, light housekeeping, and personal care
• Ensure clients’ safety, comfort, and well-being
• Report clients’ needs and conditions to the Care Coordinator
Requirements:
• Aged care certificate 3 or 4 (preferred)
• Police Check (mandatory)
• First Aid Certificate (preferred)
• Previous aged care or community support experience (preferred)
• Kindness, patience, and reliability
